Hours are current for the month up to 4 am the previous day.

Please return this, E-bill with your payment, or use this Form with Your payments.
- A charge of $0.50 is charged per hour of usage over block amounts purchased in any given plan
- Hours are calculated on calander months not billing months.
- hours from 1:00 am to 7:00am are not charged against Your account.
- Commercial accounts are also not charged for hours from 7:00 am to 5:00pm (restrictions apply)
- Accounts are E-billed on or befor Your due date, if payment is not received or arranged within 15 days a
second E-mail notice will be sent. If payment is not received within 30 days of due date the account will be
locked. Accounts will be kept in the dormant (locked) state for 1 month before final deletion.
